We work hand in hand with your staff, every step of the patient journey

From admission, all the way through discharge

Upon Admission

  • Understand patients’ special needs, pre-admission

  • Review patient charts within 24 hours of admission

  • Review facility census bi-weekly to flag at-risk patients

In Facility

  • Weekly patient visits and meetings with nurse leaders

  • Coordination with cardiology and other specialists

  • Remote monitoring of latest lab results

At Discharge

  • Smooth handoff to patient’s primary nephrologist

  • Support the safe transition back home

  • Reduce likelihood of a return to the hopsital

Our approach to consultations

  • We monitor fluid / electrolyte imbalances, along with malnutrition and cachexia, to ensure proper kidney function.

  • We review medications and do frequent medication reconciliation. Our approach involves oversight of polypharmacy and any drug-drug interactions, as well as avoidance of nephrotoxic medications. We also conduct a comprehensive pharmacokinetics and pharmacodynamic review to ensure medications are appropriately dosed.

  • We coordinate closely with other specialties like cardiology and pulmonology and others. Often comorbid conditions affect multiple organs including the kidney.

  • We train your nursing staff to report important changes in renal condition to our nephrology team, as well as on best practices in nutrition and medication management.
